Kristen will be your first point of contact to get things started, and take you through a few quick eligibility questions. There is no application and no paperwork!
Next, you will receive a call from Matt. He will gather more information and an in-person home visit will be scheduled. During the home visit, Matt will walk through your home with you, ask questions and make suggestions. Together, you will determine what eligible modifications meet your individualized needs. This is a collaborative process, we want to meet your needs.
Self-referral required: Comfortably Home requires self-referral. We need to hear directly from anyone requesting services.
